Before completing this registration form, please ensure that your school has already registered as a delegation. Your Faculty Advisor (FA) should have reached out to us regarding the delegation’s payment type. Individual delegate registration is only valid once your school has confirmed its delegation registration.

We strongly encourage you to register in coordination with your Faculty Advisor and Head Delegate to ensure consistency within your delegation.

Independent Delegate Acknowledgment

Before completing this registration form, please ensure that you are registering as an independent delegate, not affiliated with any school, institution, or organization delegation. By registering independently, you acknowledge that you will be fully responsible for your own registration, payment, and participation in the Summit.

Please note that independent delegates do not require a Faculty Advisor or Head Delegate, but are expected to follow all Summit rules, procedures, and codes of conduct as outlined by UTSRI.

1. ACKNOWLEDGEMENT OF RISKS

I hereby assume all risks of participating in any and all activities associated with the University of Toronto Student Research Initiative Scientific Research and Strategy Summit, including but not limited to risks of personal injury, illness, or exposure to potential physical or mental hazards.

I certify that I understand the nature and extent of these risks and the usage of facilities, equipment, or services provided by UTSRI. I voluntarily accept full responsibility for any loss, damage, or injury that I may sustain while participating in this Summit.

2. RELEASE AND WAIVER OF LIABILITY

In consideration of being allowed to participate in the University of Toronto Student Research Initiative Scientific Research and Strategy Summit, I hereby release, waive, and discharge UTSRI, including its executives, directors, affiliates, and sponsors, from any liability, claims, demands, or causes of action that may arise from my participation, including but not limited to negligence on the part of UTSRI or its partners.

3. INDEMNIFICATION

I agree to indemnify and hold harmless UTSRI and its officers, directors, affiliates, and sponsors from any claims, damages, losses, or expenses (including legal fees) that may result from my participation in the Scientific Research and Strategy Summit.

4. MEDICAL AUTHORIZATION

I certify that I am in good health and have no conditions that would prevent me from safely participating in this activity. In the event of a medical emergency, I authorize UTSRI to obtain treatment for me as necessary.

5. PHOTO/VIDEO RELEASE

I grant UTSRI permission to use my image, likeness, or voice in photographs, videos, or other media from this event for promotional purposes.

6. GOVERNING LAW

This agreement shall be governed by and construed in accordance with the laws of the Province of Ontario and, where applicable, the federal laws of Canada. Any disputes arising under or in connection with this agreement shall be subject to the exclusive jurisdiction of the courts located in Toronto, Ontario.

7. ACKNOWLEDGMENT AND SIGNATURE

I confirm that I have read and fully understand this agreement, and I voluntarily sign it, acknowledging that I am waiving certain legal rights.

I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS EVENT, including by way of example and not limitation, any risks that may arise from participating in the University of Toronto Student Research Initiative Scientific Research and Strategy Summit, including but not limited to personal injury or illness, death, exposure to potential hazards, including that of physical or mental nature that may arise.

I certify that I understand the nature and extent of the risks associated with the UTSRI Scientific Research and Strategy Summit and the usage of facilities, equipment, or services in association with and provided to the participants by UTSRI. I voluntarily assume full responsibility for any risks of loss, damage, or injury that may be sustained by my child participating in this Case Competition.

In consideration of allowing my child to participate in the University of Toronto Student Research Initiative Scientific Research and Strategy Summit, I, on behalf of my child hereby release, waive, and discharge UTSRI, including its executives, directors, presidents, affiliates, and sponsors from any liability, claims, demands, lawsuits, or causes of action that may be sustained from my child’s participation, including but not limited to negligence on behalf of the University of Toronto Student Research Initiative or any other parties involved.

3. INDEMNIFICATION

I agree to indemnify and hold harmless UTSRI and its officers, directors, affiliates, and sponsors from any potential claims, actions, damages, losses, lawsuits, or expenses (including legal fees) that may result from my child’s participation in the Scientific Research and Strategy Summit.

4. MEDICAL AUTHORIZATION

I certify that my child is in good health and has no prior conditions that would prevent them from safely participating in this activity. In the case of an emergency, I authorize UTSRI to obtain medical treatment for my child, along with contacting myself, if necessary.

5. PHOTO/VIDEO RELEASE

I grant UTSRI permission to use my child’s image in photos or videos of this event for promotional purposes.
